Health Monitor Service

The Health Monitor Service is intended only for workers enrolled in the Sanedil Fund, aged 40 years or over, and enables remote monitoring (using special devices provided free of charge by UniSalute) of clinical values in the case of the following chronic diseases:

  • DIABETES;
  • HYPERTENSION;
  • COPD (chronic obstructive pulmonary disease);
  • ASTHMA.

How to request the Health Monitor Service

DOCUMENTATION

To activate the service, the member must submit the following forms to Cassa Edile/EdilCassa:

  • GUARANTEE ACTIVATION FORM;
  • MEDICAL CERTIFICATE CONFIRMING THE PATHOLOGY OR THE COMPLETED QUESTIONNAIRE;

(except in the case of activation of the service independently – see point 1 of the Procedure).

  • COPY OF AN IDENTITY DOCUMENT;

(this only applies in cases where the request is submitted by e-mail).

PROCEDURE

SUBSCRIPTION VIA DEDICATED WEBSITE

Submit/send the appropriate guarantee activation form to the relevant Fund. In this case, the member will receive, subject to verification of insurance cover, an e-mail containing the link to the dedicated site, which will enable him/her to independently complete the questionnaire or upload the medical certificate and send the request to UniSalute. Subsequently, the member will receive a further e-mail with the outcome of the request.

PRE-COMPLETED SUBSCRIPTION

Submit/send to the relevant Fund the appropriate guarantee activation form, selecting in the part relating to the application for membership one of the two methods provided, i.e. by:

  1. QUESTIONNAIRE, TO BE COMPLETED BY THE MEMBER;
  2. CERTIFICATE FROM THE ATTENDING PHYSICIAN OR SPECIALIST.

The operator of the Cassa, based on the documentation provided, after verifying the insurance cover, will complete the guarantee activation procedure, sending the request to UniSalute. Subsequently, the member will receive a further e-mail with the outcome of the request.

ASSISTED SUBSCRIPTION

By coming directly to the Cassa’s public counter, the subscriber, after checking insurance cover, will be supported by an operator in joining the Health Monitor service, which can be done by filling in a questionnaire based on the information provided by the employee or by presenting a medical certificate. Upon completion of the required data entry, the subscriber will receive an email confirming their willingness to join the Health Monitor service and will be asked to enter their social security number. They will then receive a further e-mail with the outcome of the request.

SERVICE ACTIVATION

If the request is successful, the member will receive a free telemonitoring kit on loan from UniSalute. Through the SMS service, delivery times and methods can be tracked and possibly changed according to the member’s preferences.
Once the KIT has been received, the member will be contacted by a UniSalute operator to be guided through the configuration of the phone app and devices provided, and to start monitoring.
